The Health/Demographic section is where you can record relevant medical history and risk factors for the member. This helps Zemplee’s system contextualize alerts and provides deeper clinical insights to caregivers.
🩺 What You Can Record (Optional, but Recommended)
Chronic Conditions (e.g., Dementia, Diabetes, Parkinson’s)
Behavioral health indicators
Mobility level
Communication preferences
Cognitive status
Language and cultural notes
This information supports more personalized care and better interpretation of behavior-based alerts.
⚠️ Optional: Enable Fall Risk Scoring
Zemplee offers a Fall Risk Scoring Tool to help identify patients at higher risk of falling based on a combination of:
Demographic inputs (e.g., age, mobility)
Health status
Ongoing sensor monitoring data
You do not need to enable fall risk scoring, but if you choose to activate it, additional fields will become required, such as:
Use of mobility aids
History of previous falls
Gait or balance issues
Medication types impacting balance
Once activated, Zemplee will generate an automated Fall Risk Score, which updates over time as the member’s activity patterns and health inputs change.
